Do we have to send in our receipt with the settlement papers?

I bought the v710 the day it came out. I believe I paid around $400 for this when it first came out. Can I get the $400 back, and do I need to send them my recipt? Thanks in advance...

Also, what about the TransFlash cards we purchased? Does that count as an accessory? There is no point in having it if I don't have the phone anymore. I guess we will probably have to bit that cost..
