Someone who is in Chicago, LIKE ME, can PTT someone in Florida, like my cousin. We did so with Nextel. When they use the term "walkie talkie", they dont mean literally walkie talkie's like you used to play with as a kid. The walkie talkie feature uses the same network as M2M. Just because the walkie talkie is usually heard on speaker doesnt mean thats the only way to walkie talkie on a phone, you could always turn off the speaker and use the phone as if you were making a standard call.
As far as eavesdropping, no, they cannot. And if they can, they could also do the same with your standard mobile call.

The on-network is nation-wide and available anywhere NEXTEL has service (typicially in metro areas).
The off-network (or called Moto-Talk) is available anywhere but only on certain NEXTEL phones. It's range is limited to only a few miles and is affected by outside influences such as buildings and foliage.
As to it's security, on-network PTT is very secure and can't be monitored off the air by someone unless they have a very expensive ($30K) service monitor.
I haven't had the opportunity to research off-network PTT privacy, but being it's spread-spectrum I'm sure you not going to have some scanner buff eave...

If that was possible, Nextel would be able to PTT Verizon or Sprint, vice versa and what not. But thats not possible and I dont think it will be with Cingular, at least not for a long time. Im really not sure how you'd PTT a land line either...
