settlement form returned, a little confusion here....
I opted for #2 in section C , ie: cancel service without penalty & a refund for phone and/or accessories.
I also checked below section C , "yes" I paid an early cancel fee.
I now realize these are for current or non current. If you check "yes" there is nothing said about a phone refund only in #2 is this mentioned.
I've already been charged for leaving Verizon & I want to be reimbursed fee's & phone cost.
How does an ex Verizon accomplish this ?

I got a letter stating I was no longer a customer, but the account name was just switched, so I corrected the spelling of the new name and sent it back...hopefully it will work now..
