I have an iPhone 4 and have had a case since day one..have never had a dropped call. Shocking to say about AT&T, but they are great in my area..
The reception would only drop if you held the phone over that particular band without a case. They didn't necessarily fix the issue, (as it's part of the design) but the 4S didn't have the issue.
