A PUK code is not a SIM Unlock code. It is given to you by the operator to unlock your SIM (not your phone) if you enter your PIN incorrectly 3 times and lock yourself out of your SIM.

If your phone is SIM locked and your insert a SIM from a network which the phone is not SIM locked to, you will receive the message "SIM rejected".

It is more likely that your phone is attempting to register on a network that does not allow your SIM. (i.e. trying to use a Cingular SIM to access, or roam on, T-Mobile's network).

To verify if this is the case, manually scan for networks and select the one you want. See if it is accepted (go to settings->networks->manual).
