This is the FCC filing Apple submitted today. it is current for the Verizon CDMA iPhone4
The FCC doesn't test them, it is up to the manufacturer to have the phones tested at labs that are certified by the FCC. Then they submit the test info and all info about the phone to the FCC for approval and to keep on file.
If they make changes to the phone, they have to get that part of it tested and approved again and will file additional documents with the FCC for any updates they do.
The redesign on the antenna was for the CDMA signal and frequencies, which will require a different antenna from a CDMA phone, as they use different frequency bands. I doubt there will be any changes before the phone is released in a few weeks, however, they may make changes later if there are a lot of signal problems with the phone.
