The end of Assassin's Creed: Embers. It's a short animated film that takes place after Revelations and it's the end of Ezio's story. If you've played the previous games you've been with this character and seen his life from a naive teenager whose parents are killed all the way up to a wise old man training the next generation of Assassins.
At the end of Embers he's sitting on a bench in the same plaza where his family was executed, watching his wife and children happily, and he passes away from the cough/sickness/whatever that's been plaguing him for the entire film. It's very well done and very subtle, and super sad.
