My (California resident) experience in a nutshell:

COVERAGE:
> ATT & Verizon have the largest coverage areas. ATT has a slight edge over Verizon in rural coverage area.
> Nextel & Sprint each come in second. Nextel is perfectly good in metro areas and main highways, not as good as ATT or VZ in remote or very rural areas. Although Nextel's iDEN and Sprint's CDMA networks are roughly the same size now, before the merger Nextel had better coverage than Sprint. But Sprint's CDMA network is receiving more investment now than the Nextel side is.
> T-Mobile is worst; forget it.

SIGNAL QUALITY (sound quality, lack of dropouts):
> Verizon is best.
> Nextel is second
> Sprint is third
> ATT is fourth
> T-Mobile is worst
