For the countries I checked (Japan, India), their international rates were only slightly better than roaming on a domestic (US) carrier.
Roaming in the UK was better, but you can only get the best rate by spending an additional $12.99 a month.
The advantage of having a local number on the same SIM as a US number is offset by the fact that a traveler would probably not use the service normally in the US.
A far better solution is to use a foreign country SIM in an unlocked GSM phone while traveling. You get a local number in the country you are traveling, the best rates for local calls and in many cases low-cost international calling rates. You can also set up Skype or Gizmo for a US based number and have that number forward to your foreign country number.
