If you think those adapters cost $5 to make you're delusional. I remember when I ran my cellphone store I bought that kind of stuff wholesale for $1, and those guys were making bank. I'd imagine it costs somewhere between $.01 and $.05 to make one. It wouldn't be much of a loss to include it in the box, but then Apple couldn't completely stick it to their customers.
