As far as I'm concerned, T-Mobile has the best unlock policy. As long as you own the device, they will give you an unlock, no matter if you are under contract or not, no matter if the device was received with a discount or not.

If you paid full retail price for a device, you qualify to have the SIM subsidy unlock code provided if you meet the following requirements:
The device must have been purchased at a T-Mobile retail store or authorized T-Mobile retailer.
The device must be paid for completely. You must be able to fax the proof of purchase to T-Mobile.

Now I bought my Amaze 4G in a T-Mo store without subsidy last November, on Black Friday, but I don't think I still have the receipt for it. I am on a prepaid 4G plan in good standing and I don't see why I have to go through all those hoops to get my phone unlocked. That whole faxing thing reminds me the conditions of those stupid mail-in rebates that are obviously aimed to stop...

Unable to keep your receipt is not T-Mobile's fault, you need to provide proof you actually own it if you bought it at a corporate or authorized retailer or else who's to say that someone won't just steal them then get the subsidy code directly from the carrier? Also to add T-Mobile does give the subsidy to people who purchased their phone on contract 6 months after they bought their phone even after it was given to them on discount.

That is silly. The phone has all kinds of identifying info (IMEI, IMSI #, etc.) that should be enough to ascertain by the carrier if a phone is stolen or not. Why should one keep a receipt for it forever? Luckily though, after some search I did find the original box that the phone came in and the receipt is in it. I hope that would be enough to take to a T-Mo store instead of faxing it somewhere when I don't even have a Fax. Who needs Fax anymore anyway when one could attach a scanned copy to an email?

t-mobile's policy is very specific.
retail stores do not complete sim unlock requests, even if you have the receipt.
you must complete it by calling customer care, or by contacting live chat on the contact us page on their website. you must fax it to the number provided - that is your only option.

I have a Mytouch 3g slide which I purchased used. I was on monthly 4G prepaid for 2 months and they provided me the unlock code when I told them I needed it because I was traveling overseas. Here's all they require from their email.
1. Must be active for 60 days
2. Must have a balance of at least $10.00 or have had a refill placed on the account within the last 30 days

Following bd8371's example, today I called up T-Mo to unlock my phone and indeed, there was no need for faxing anything as they could tell from my record I was legit and now I am waiting for the email with the unlock code. Cool!
