I have a Mytouch 3g slide which I purchased used. I was on monthly 4G prepaid for 2 months and they provided me the unlock code when I told them I needed it because I was traveling overseas. Here's all they require from their email.
1. Must be active for 60 days
2. Must have a balance of at least $10.00 or have had a refill placed on the account within the last 30 days
